Suboxone - A New Treatment Paradigm Part Two

Drug Abuse Addiction Comments Off

In Part One of this article I described the problems with traditional treatment of opiate addiction. Suboxone is a revolutionary alternative.

Suboxone consists of two drugs; buprenorphine and naloxone. The naloxone is irrelevant if the addict uses the medication properly, but if the tablet is dissolved in water and injected the naloxone will cause instant withdrawal. When suboxone is used correctly, the naloxone is destroyed in the liver shortly after uptake from the intestines and has no therapeutic effect. Buprenorphine is the active substance; it is absorbed under the tongue (and throughout the mouth) but destroyed by the liver if swallowed. There is a formulation of buprenorphine without naloxone called subutex; I have used this formulation when the patient has apparent problems from naloxone, including headaches after dosing with suboxone. I have also treated addicts who have had gastric bypass, where the first part of the intestine is bypassed and the stomach contents empty into a more distal part of the small intestine. In such cases the naloxone escapes first pass metabolism, the process with normal anatomy where the drug is taken up by the duodenum and transferred directly to the liver by the portal vein, where it is quickly and completely destroyed. After gastric bypass naloxone can be taken up by portions of the intestine that are not served by the portal system, causing blood levels of naloxone sufficient to cause brief, relatively mild withdrawal symptoms.

Buprenorphine has a ceiling effectthe narcotic effect of the drug increases with increasing dose up to about one or two mg, but then the effect plateaus and higher amounts of buprenorphine do not increase narcosis. The average patient usually takes 12-24 mg of suboxone per day, and quickly becomes tolerant to the effects of buprenorphine (buprenorphine does have significant narcotic potency, but the potency usually pales in comparison to the degree of tolerance found in active opiate addicts).. The opiate receptors in the brain of the addict become completely bound up with buprenorphine, and the effects of any other opiate medication are blocked. Once the addict is tolerant to the correct dose of suboxone, the buprenorphine that is bound to their opiate receptors reduces cravings and prevents the effectsand so the use–of other opiates. Suboxone is very effective in preventing relapse; the choose to use issue is effectively removed by the fact that use would require the addict to go through several days of withdrawal in order to remove the receptor blockade and allow other opiates to have an effect. Given addicts attitudes toward withdrawal, the appeal of this choice is quite low. The only real problem with suboxone treatment relates to specificity. With suboxone, the addict stays off opiates, but there is nothing to prevent the substitution of alcohol. On the other hand, naltrexone reduces alcohol cravings by blocking opiate receptors, and it is quite likely that suboxone, through its similar mechanism, will reduce alcohol cravings as well. Such an effect has been reported to me by a number of suboxone patients, but has not been reported in the literature at this point. The suboxone patients who move from one substance to another will likely require an approach that demands total sobriety. But for pure opiate lovers, other benefits of suboxone are that only mild (and possibly medicated) withdrawal is required to start treatment, the drug is usually covered by insurers, prescribing restrictions are minor, and there are fewer stigmas associated with maintenance than there are with methadone.

As I stated in part one of this article, I predict that suboxone will eventually be the standard treatment for opiate addiction, and will change the treatment approach for other substance addictions as well. My only reservation with this statement is that it is unclear how the current recovering community will respond to patients treated with suboxone. If suboxone patients are rejected by the recovering community, what will be the long-term outcome of their addictions when the substance is removed but the personalities and issues remain untreated? Is it a given that all addicts have a disease that requires group therapy? As things stand now, addicts maintained on suboxone are often referred for addiction counseling. But the exact message to deliver with counseling is debatable. In many ways, a patient maintained with suboxone becomes similar to a patient with hypertension treated for life with medicationthe underlying problem persists, but the active disease is held in remission. If the uncontrolled use of opiates is effectively treated, is that enough? Should counseling be focused on removing the shame of having the disease of addiction, and on encouraging the treated addicts to get on with their normal lives? Or should we continue to see addiction as a consequence of a deeper problem or faulty character structure, which requires groups and meetings if one hopes to become normal? Unfortunately the use of suboxone runs counter to successful adoption of sobriety through 12-step programs, which in the first step require acceptance of the fact that the addict is powerless over the substancethat there is no amount of will power that will allow the addict to control the deadly effects of the drug. By using suboxone the addict may develop the impression that he/she has control, particularly if suboxone becomes popular on the street for self-medication of withdrawal.

Before suboxone, the only option for opiate addicts was to lose a sufficient number of thingsfamily, employment, freedom, healthto cause them to accept treatment and recovery. Only a small fraction of addicts recovered, and only after significant lossesand relapse rates were high. Suboxone is an amazing breakthrough; one that for the first time allows treatment of addicts early in the course of their illness, and that reliably induces remission in most patients. But there are some things to be concerned about, that have the potential to reduce the effectiveness of this amazing new drug and treatment approach. First, some insurers demand that the drug be used only short-term, in some cases for only three weeks! This requirement totally misses the nature of addiction, and ignores the known high relapse rate after short-term use of suboxone (and why wouldnt it be high?). Some physicians use the medication in this short-term way; hopefully the motivations for this ineffective treatment method are not related to the limits placed on the numbers of maintenance patients per physician. Other physicians will transfer their attitudes toward opiate agonists to the use of suboxone, and place constant downward pressure on the daily dose of suboxone. This approach is not appropriate with suboxone; the value of the drug requires adequate dosing to achieve the long half-life and repression of cravings. At doses of less than 8 mg, suboxone becomes more similar to a pure agonist; one might as well be giving small doses of hydrocodone to prevent withdrawal. There is no reason beyond drug cost to reduce the dose, as tolerance is limited by the ceiling effect that occurs with relatively low doses. In other words, higher doses of suboxone do not result in eventual higher degrees of withdrawal. Another issue is that the medication is sometimes prescribed carelessly, without emphasizing the need to dose once per day. Patients left to their own devices will start using the medication multiple times per day as a prn medication, and will remain in the same addiction behavior that brought them to treatment. Once per day dosing is important because it allows the addictive behavior to be extinguished over time. Initially patients will have increased anxiety as they lose the distraction and placebo effect of frequent drug use. But over time the anxiety will fade, and the huge void left by the removal of addictive obsession will allow the development of relationships and other positive character traits that were forced out by their addiction.

Given the time pressures and payment structures of modern medicine, suboxone may eventually replace residential treatment as a more reliable, less costly alternative. I believe that the time has come to replace the recovery model with a new remission model, which allows treatment of a much higher percentage of users at an earlier stage of disease. With time, will we find analogous agents that provide a low level of intoxication in return for receptor blockade? While not likely with alcohol, such an outcome is certainly within the bounds of imagination for cocaine, benzodiazepines, and barbiturates. While it is true that daily use of a partial agonist would represent a reversal from our current approach where all intoxicating substances are to be avoided, it is also true that the current approach has no bragging rights based on outcome. Finally, perhaps the adoption of a remission model will lessen the time until opiate and other addictions carry as much moral stigma as hypertension or diabetestwo other diseases that are generally treatable, but that require long-term use of medications.

Does anyone say as a child they would like to grow up to be an addict one day? No one wants that type of life but millions of people find themselves struggling with addiction every day.

It\’s a devastating condition that wreaks havoc on families and people from all walks of life. It\’s not just bums on the street. It\’s doctors, lawyers, teenagers and soccer moms.

Drug addiction is at staggering levels and many people struggling with this feel they have no hope of recovery. Most treatment centers and counselors here in the US follow the 12 step notion that addiction is an incurable disease that you are powerless against.

Where then does that leave you? If you have no hope of recovery are you then doomed to a dreadful life?

There is substantial evidence supporting the connection between your mind and body. If your mind can think and believe something then you can actually retrain your subconscious into reinforcing that thought into actions.

So for example, if you can get the thought that drug addiction recovery is possible into your subconscious, your brain will actually begin working towards that goal.

That\’s such an important statement because many people laugh off the power of thought but time and time again it\’s been proven to work.

Every great invention was developed in someone\’s mind. Our brains have incredible power and can impact every cell in our body. It\’s important to fill them with positive thoughts especially when overcoming drug dependency.

It\’s not just a matter of will power when thinking of drug addiction recovery, it\’s harder than that. Non-addicted individuals may feel that the cure lies in just not using again but we know it\’s not that easy.

We can\’t just quit because the drugs and alcohol are our means of coping with life. It may be to escape the stress of an unhappy home or to forget about painful childhood memories. Whatever your reason, you are using drugs or alcohol as a medication.

The real solution lies in retraining your brain to use new, healthier coping mechanisms and get in better habits of a drug/alcohol free lifestyle.

If you feel you cannot stop drinking on your own, talk with your doctor about treatment centers. Your insurance may cover part or all of the care you receive. If you have an Employee Assistance Program (EAP) at work, you can discuss your concerns in a confidential setting and find the help you need. You can also check for local resources in your city.

You also want to address the emotional and habitual side of the problem. Many people find support groups like Alcoholics Anonymous (AA) help them resist the urge to drink. Whenever you watch your favorite CSI TV show, you may have heard of the victim being jacked up on mind-altering drugs such as hallucinogens that were found in a toxicology screen.

What are hallucinogens and how do they differ from hallucinations and delusions?

In this article, I will attempt to explain what each of these terms is.

Hallucinogens are drugs responsible for changing one\’s perceptions and mood, contribute to delusions, and promote hallucinations. Delusions are false beliefs that have very little basis in reality. For instance, one may believe that his dead mother is coming back from the grave when in reality she has been dead for many years.

Hallucinations are false experiences that affect our senses. They are not real and may affect all of our senses such as sight, sound, smell, taste, and touch. For instance, one could claim to feel ants crawling all over his skin when in reality there ARE no ants whatsoever. This is an experience that has affected a person\’s touch.

The most commonly encountered hallucinogenic drugs occur naturally from plants such as marijuana, mushrooms, and peyote. Other types of hallucinogens are synthesized in a laboratory. They include LSD, PCP, and STP.

Hitting on Mary Jane

Thus far, the mildest and perhaps the most frequently used hallucinogenic drug is marijuana. Marijuana has several nicknames, which include Mary Jane, pot, and grass. It is a derivative of a plant called Cannabis sativa. The active ingredient sought after in this plant is a molecule called tetrahydrocannabinol (THC). THC is found in marijuana at a concentration between 2 and 6 Cannabis sative plant is called hashish which contains about 12 percent THC.

The human body catabolizes THC into one of several products, the most important being 9-carboxy-tetrahydrocannabinol (9-carboxy-THC), which shows up in one\’s urine. When somebody is subjected to a urinalysis, the drug test will be positive for that the person has been using marijuana lately. The drawback of this drug test is that someone can still test positive for the presence of 9-carboxy-THC in minute levels if he is exposed to second hand marijuana smoke.

You may have seen the CSI: Miami episode in which Eric Delko was asked to do a urinalysis that yielded a positive result. Later, his sister, Marie, attested that Eric is not a marijuana smoker but rather she is, and Eric was only around when she blazed up to self-medicate herself since she suffers from cancer.

Mushrooms and Cacti

Possession of marijuana is illegal in many states but the possession of mushrooms is not. If somebody is caught with mushrooms, it is up to a toxicology laboratory to determine if psilocin and psilocybin, the psychoactive chemicals found in mushrooms, are present for the mushrooms to be considered illegal.

Peyote is the Spanish word that refers to a small Mexican cactus. The sought after ingredient in peyote is a molecule called mescaline, which is a hallucinogen. Many Native American tribal cultures have used peyote in social gatherings for centuries. The external surface of the peyote plant is covered with small round bumps called peyote buttons. These bumps are compartmentalized like an orange. Every compartment contains a cottonlike tuft within.

Experimentally, gas chromatography (GC) or thin layer chromatography (TLC) can identify the presence of mescaline. Unlike mushrooms, further testing is not needed because possession of the peyote plant, just like marijuana, is considered illegal.

Synthetic Hallucinogens

There exists a wide array of laboratory produced hallucinogens. The most common is lysergic acid diethylamide (LSD) and phencyclidine (PCP). PCP is sometimes known angel dust. LSD is very strong. As little as 25 micrograms can produce a high that lasts up to 12 hours. Hallucinations associated with this popular 60\’s drug are vivid. Even though LSD is not fatal, there have been cases in which LSD users have harmed others or themselves because of their changes in perceptions. A chemical test called the Van Urk Color Test can test for the presence of LSD.

A forensic toxicologist can use either blood or urine to check for the presence of PCP. For PCP users, a urinalysis can yield a positive result for up to a week after the last use.

The next time you watch your favorite CSI TV show, you will have a better understanding of the drugs and the drug tests used to determine if a user is positive for a certain hallucinogen.

The drug rehab business is undoubtedly a for profit business, and as such when someone sees a marketing niche as yet unfilled. You can bet they\’re going to start filling it; whether it\’s truly needed or not. It can therefore be very tough to know what you need, who to trust and where to go.

Do women need a distinct form of rehab, or do they recover just as well in a general adult rehab program?

They do.

Well, they do need to recover in a rehab that meets their unique needs anyways. If a general adult rehab can offer gender specific therapies and the treatments that women need to better their unique obstacles to recovery, then that can work fine as well.

Women suffer greatly through addiction or alcoholism, they suffer more than men do in fact; but thankfully, and as if to make up for this unfair reality, when women get the therapies and treatments they need, they have lower relapse rates than men do. Women need the same kinds of therapies than men do. They need 12 steps group meetings, they do very well in group therapy and they benefit as anyone does from individual sessions with a therapist. But women need more than this alone.

Gender specific therapies

Women bring a unique personal history into a drug or alcohol rehab, and they have in many cases endured some very different challenges on the dark journey through addiction. Without having their needs met, and their challenges understood, they cannot hope to make a full recovery.

Sexual abuse therapy

Studies show that about half of all women entering into addictions treatment have suffered sexual violence or assault, either during the period of abuse, or as a catalyst to that abuse. Women who have experienced sexual assault need sexual trauma therapy, to start to work through those issues lingering from abuse. A rehab program that fails to deal with a legacy of abuse offers women little hope of a full recovery.

No rehab can heal the scars of trauma in a month or more, but therapies that unearth lingering issues and start the healing process give women the tools they need to start healing, and to start to move beyond assault.

STD\’s

Women entering into rehab are twice as likely to suffer from an STD as are men. Any lingering and unresolved health challenge hampers recovery, and since health problems can provoke relapse, failing to acknowledge and deal with STD\’s and other health challenges leaves women very exposed to relapse.

Children

A loft of women bring enormous shame and guilt into recovery. No one can get better until they can forgive themselves for the sins of the past, and family reconciliation therapy helps women to understand that although they may have done some very irresponsible things while under the power of addiction, they are not what they did, and they can do better in the future. Parenting and familial reconciliation therapy ensures that women gain the self awareness and tools to move forwards and do better, without getting dragged back by the shame of the past.

Women only rehabs

Women do not need to go to a women only rehab, although many do choose to and this is an equally valid option for recovery, but they should never consider a rehab that does not understand their challenges, and that does not treat the concerns of their gender with respect and with treatments that work. Even though drug addiction is a complex disease, assigning a definition for that disease is actually quite simple. Drug addiction is, in its simplest form, defined as the continued compulsive use of drugs in spite of adverse health or social consequences.

The definition of drug addiction should also include the fact that it is a state of heavy dependence on a specific drug. Generally, its thought of as a physical dependence, but in actuality, there is a psychological addiction as well as an emotional dependence. Some say that it is compulsive and pathological drug use or substance dependence.

Drug addiction is also a behavior disorder as the initial drug use is generally done as a reaction to a behavior or situation that seems out of control to the user. The user seems to have no other choice but to use that drug because of the bodys dependence on the fact that the drug will remain present in the system.

A distinction should be made between drug use and drug addiction. It is possible for people to use drugs without becoming addicted, but that is generally limited to prescription drug use. People who take drugs to cope with a physical ailment do so to control pain or the adverse effects of a physical condition. Those drugs improve the quality of life for the sufferer, but they are not necessarily addicted to the drugs.

To be dependent on a drug or addicted to it the user must take the drug regularly, and they experience unpleasant symptoms if they stop using the drug such as nausea or sleep disturbance. Substance abuse or addiction is present when a person uses a drug despite the harm that it causes to their system.

There is a difference between drug abuse and drug addiction. Drug abuse occurs when a person uses excessive amounts of a drug at one time or frequently. Drug addiction is present when a person cannot stop using the drug and stopping the drug seems like an impossibility.

Of course, no definition of drug addiction is complete without noting that it is both a physical dependence as well as a psychological one. Physical dependency occurs when a drug has been used habitually and the body has become accustomed to its effects. Psychological dependency is present when the drug is used habitually and the mind has become accustomed to the effects the drug produces.

When drug addiction affects a persons life, the definition of that drug addiction becomes a bit less important than the reasons why people use. However, it can help multitudes if a definition is specified so that a better understanding of drug addiction can occur.

As children grow older, one thing they can count on is being asked to take tests. It\’s just part of growing up.

Teachers want to know if students are learning. The state wants to know that roads aren\’t filled with drivers who lack the knowledge and skills to operate a vehicle safely on public roads. Parents want to know if their kids are doing drugs, and they are turning to home drug testing kits to find out.

We\’ve all grown up with report cards and drivers licenses as proof of our ability to pass these tests. However, with a new way to privately test kids at home for drug use, more and more parents want to see proof.

Home drug testing kits give parents instant results with lab-quality accuracy and give kids a socially acceptable excuse to resist peer pressure. The words \”No thanks, my parents test me\” stop pushy peers in their tracks.

Critics often claim that home drug testing is a violation of trust. If we could just take a child at his or her word, why would we ever need to conduct a math test or a driver\’s test or ever look at a report card?

If anything, I contend that it is more important for parents to look at the results of home drug tests than those of a spelling test, for example, since kids aren\’t likely to lose their limbs, lives, or freedom (by going to jail) simply because they misspelled a few words.

As long as expectations have been communicated and rewards as well as consequences have been spelled out in advance of test results, kids should feel no differently about parents looking at their drug test results than they do about showing their report card or driver\’s license.

Good students are proud to show off their good grades, and kids who don\’t do drugs should be proud to show their parents that they aren\’t intimidated by peer pressure and can make healthy choices in spite of it.

Good schoolteachers embrace the responsibility of showing up for work each day focused on how to help their students learn. They teach and test. They don\’t seek to be friends or buddies with their students and are more interested in their students\’ learning than they are in their students\’ opinions about how \”cool\” they are.

Good parents are good teachers too. They just have fewer students and those students just happen to be their children.

Rapid Drug Detox - Six Reasons Why It Doesn\’t Live Up To The Hype

For nearly two decades, rapid opiate detox programs have been touted in the media and on the Internet as \”the big new detox thing\”, and the promotional claims have continued. But after 20 years, this often inordinately expensive and frequently dangerous procedure is still not mainstream, because it has real skeletons in its closet — rapid detox has killed people who might have survived a modern medical drug detox.

Rapid opiate detox refers to various approaches that use \”opiate antagonists\” — drugs that rapidly block the \”opiate receptors\” in the brain, swiftly bringing about the withdrawal symptoms expected for the substance being treated, and for the severity of the patient\’s dependence.

Since \”cold turkey\” withdrawal symptoms can be severe, the procedure is performed under general anesthesia lasting several hours. The theory trumpeted by rapid detox practitioners is that upon awakening, patients are \”refreshed and free of withdrawal symptoms, and no longer crave drugs.\” In fact, recovery can take days, and some patients have continued to experience withdrawal symptoms weeks later, and others have complained of nausea for months.

Unfortunately, the claims for an safe and instant cure don\’t agree with reality:

1. General anesthesia carries considerable inherent risks itself. Anesthesia is directly responsible for thousands of unexpected deaths every year, and there\’s no reason to expect it to be any less risky during rapid detox, and reasons for it to be even more so.

2. Even though the patient is anesthetized, the body suffers enormous stresses as it experiences severe and traumatic withdrawal symptoms. One doctor commented that a patient had to be strapped to the table because their body was \”flopping around like a fish out of water.\” The effects of such stress cannot be predicted, and could have longer-term health consequences. Physical recovery can take days, weeks or months.

3. Many patients complain of continuing withdrawal symptoms long after their rapid detox. They may have withdrawn from opiates, but are often given one or more medications to help deal with the stress and pain of the procedure, which is basically continuing a dependence on drugs, as well as risking interactions with other drugs if the patient relapses.

4. The drugs used for rapid detox themselves carry certain risks, so much so that they require additional medical training to legally prescribe them. The training has been called less than perfect by some critics, for several reasons:

a. If a patient has unknown or unreported medical conditions or allergies, a rapid drug detox drug, or the anesthesia, or both together, could be devastating. The training, skill and experience of the doctors is vitally important. For example, seven deaths were reported in New Jersey because patients had underlying heart conditions, or took cocaine some time after the treatment. Several deaths were reported in Michigan following rapid detox treatment, resulting in the suspension of the physicians\’ licenses by the state\’s Attorney General.

b. A National Institute of Drug Abuse (NIDA) report warned of complications with the procedure that are \”unacceptable\” for an unproven treatment method. The report went on to say that patients may be at risk of choking or cardiac complications when given large quantities of detox drugs in combination with anesthesia.

c. No two people are the same, and no two addictions are the same — everyone\’s unique DNA, metabolism, level of health, addiction history and habits mean that any drug detox — not just rapid detox — is a unique situation. None of this is provided in the additional training.

d. Rapid detox — and indeed many drug detox programs in general — usually fail to take these critical factors into account, cheating patients of the chance for the complete care they should receive.

5. People who complete rapid drug detox are statistically more likely to overdose if they relapse — and the number that relapse is high. The drugs used for rapid detox seem to set a person up to be highly sensitive to opiates, so a former opiate addict who relapses may think he or she can consume the amount they used in the past, which now could be fatal.

6. Finally, rapid drug detox does absolutely nothing to rehabilitate an addict\’s life. As it said in the NIDA report mentioned earlier, \”detoxification is not a cure for opiate addiction.\” Drug detox is only the gateway to full drug rehab which addresses the underlying reasons for any addiction.

After two decades, too many people have been misled into accepting the idea that rapid drug detox means the end of addiction. Unfortunately, many rapid drug detox completions wind up back in drug detox again, and most take advantage of a traditional medical drug detox program offering personalized care and a far greater level of safety.

Take a trip. If you calculate all the money you would have spent on your addiction in a year, had you not entered recovery, you can invest that money in some fun. Take a trip somewhere you\’ve always wanted to go. Perhaps it\’s a safari in the jungles of Africa or a visit to the ski slopes. Choose a destination and then spend your time there enjoying every minute of it. This is a productive use of your finances and will provide you with positive memories rather than the negative experiences that result from addiction.

One of the challenges of traveling is avoiding temptation. If your weakness was found in a bottle of liquor, ensure that you aren\’t putting yourself in a situation that encourages you to take a drink. Many of the all-inclusive vacation packages include all the alcohol beverages you can drink. Avoid this and instead opt for a week in a mountain cabin, canoeing and hiking through the wilderness. Tempting yourself unnecessarily isn\’t going to help in the long run at all. Instead plan a trip that encourages you to stay clean.

Next time you think about picking up a drink, drug or a cigarette, remember that the cash you\’re spending could be used for a better purpose. There\’s a whole world for you to explore but spending money on substances will hinder that opportunity.

Seven years of recovery is a tough road to travel. Kim loves the life she lives now, but this sentiment certainly didn\’t always ring true. \”I had it all, a husband who loved me, two beautiful children, a big house, a great job. I started using drugs when a doctor prescribed painkillers for a back injury that happened while skiing.\” From there, Kims life went on a downward spiral, until she found a successful drug rehab program.
